Effect of Soy Protein Containing Isoflavones on Cognitive Function, Bone Mineral Density and Plasma Lipids in Postmenopausal Women

175 women were randomly assigned to receive 25.6 g of soy protein containing 99 mg of isoflavones (52 mg genistein, 41 mg daidzein and 6 mg glycetein) or total milk protein as a powder daily for 12 months.  This was a double-blind randomized trial and did not support the hypothesis that the use of soy protein improves cognitive function, bone mineral density (BMD) or plasma lipids in healthy postmenopauseal women when started at the age of 60 years or later.  Interestingly they did find that  BMD of the hip was significantly higher in the soy group, however it was only 1 comparison among 13 BMD measurements.  In reviewing this article one needs to wonder why they would compare to total milk protein. Clearly this is a very poor control, and it would have been significantly more substantial work if they had compared to a true control, ie a placebo pill, this was not the case as there are outstanding bone supporting nutrients in total milk protein. Thus, I find this to be a critical flaw in this manuscript.  Nonetheless, it has been published and it is important to note their findings. 

 

Overall take on this, do not give up your soy protein, this is one study and the control was not really a control, it was essentially another treatment arm.

 

Sugar-Sweetened Beverages, Weight Gain and Incidence of Type 2 Diabetes in Young and Middle-Aged Women

Sugar-sweetened beverages (soft drinks and fruit punches), contain huge amounts of simple sugars and likely contribute to weight gain and an increased risk of type 2 diabetes.  This study was conducted from 1991-1999 among women in the Nurses health Study II. The analysis included 91,249 women free of diabetes and other major diseases in 1991. The weight change analysis included 51,603 women for whom complete dietary information and body weight were ascertained in 1991,1995 and 1999.  They found that higher consumption of sugar-sweetened beverages is associated with a greater magnitude of weight gain and an increased risk for development of type 2 diabetes in women.

 

Type 2 diabetes affects 17 million Americans. The incidence is increasing significantly with the increase in obesity. Soft drink consumption increased 61% from 1977-1997.  Soft drinks are the leading source of added sugars in the US diet.  Additionally, cola drinks have a caramel coloring which is rich in chemicals (advanced glycation end products) that are implicated in the increase in insulin resistance and inflammation.

 

Interestingly, fruit juic consumption was not associated with diabetes risk in this study, which suggested to the authors that naturally occurring sugars may have different metabolic effects that added sugar. Fruit juices generally have a lower glycemic index (lower likelihood of rapidly increasing blood sugar) than sugar-sweetened soft drinks and fruit punches.

 

Lastly they concluded that one 12 oz can of sugar-sweetened soda contains 150kcal and 40-50 grams of sugar (that is a HUGE amount of sugar). If you add this to a typical US diet, this could lead to a 15 lb weight gain in one year!

 

Moral of this story: Be very careful what you drink, the sugar-sweetened drinks are potentially very dangerous to you. We are better off drinking water or possibly fruit juices instead.

 

Effect of Vitamin D on Falls

Vitamin D supplementation reduces the risk of alls among the ambulatory or institutionalized older individuals with stable health by more than 20%.  This was a meta analysis study, which means that the authors looked at many studies to come up with their conclusions.  The authors looked only at double-blind randomized controlled trials of vitamin D in the elderly population.

 

Falls occur in 30% of adults 65 years or older and 40-50% in people over the age of 80. The likelihood of significant injury increases exponentially once an elderly person sustains a fall. The cost of these falls was 20.3 Billion dollars in 1994 and is predicted to increase to 32.4 Billion in 2020. Thus, if there would be a way to reduce the risk of falls, this could have a huge impact on the health of people as well as health care costs.

 

It was suspected that the moderate protective effect of vitamin D on fracture risk was attributed to bone mineral density improvement. However, it has been suspected that vitamin D may also directly improve muscle strength, thereby reducing fracture risk through fall prevention. The physiology is fascinating and is briefly outlined: 1,25 hydroxyvitamin D (the active vitamin D metabolite) binds to a receptor in muscle tissue, leading to improved muscle function. There was further improvement when the vitamin D was combined with calcium (but use a chelated calcium ie calcium glycinate, citrate or gluconate, but not carbonate).

 

Prostate cancer.

Flaxseed has been shown to reduce prostate specific antigen (PSA) from 8.5 to 5.7 on average. Demark-Wahnefried showed that flaxseed slowed the growth of prostate tumors in mice. This author speculated that the lignans (from flaxseed) could be acting as a phytoestrogens which would slow prostate cell growth or they could bind to testosterone and promote its excretion (lower testosterone slows prostate cell growth).  Interestingly, there is conflicting data regarding alpha-linolenic acid (ALA) which is an omega-3 fat found in meat, vegetable oils, flaxseed and other foods. There are some studies that have shown an increase risk of cancer in men consuming more ALA. However, it is felt that meat sources of ALA may not be beneficial whereas vegetable sources of ALA may be beneficial… this is one of the difficulties of science…sorting out fact from fiction, reality from hype.  Generally speaking a diet rich in fruits and veggies is much better for us than a diet rich in meat and dairy products.

 

Other factors…too much calcium for men ie above 1500-2000 mg/day has been shown to lower vitamin D in the blood (taking extra vitamin D did not help) and less vitamin D facilitates cell proliferation (the way a cancer grows).

 

Green tea, low fat diets, selenium, tomatoes-lycopene, vitamin E and zinc are all likely to be important.

Lemon-flavored cod liver oil and a multivitamin-mineral supplement for the secondary prevention of otitis media in young children:Pilot research.

Middle ear infections are one of the most common childhood diseases. As an ENT surgeon, I see many patients each day with this particular problem. The conventional western medical paradigm, recommends treating each infection as it comes (with an antibiotic). After you have reached 4 or more infections in a year your child is a candidate for tubes in the ears. This is one of the most commonly performed procedure in this country, and while effective, if you can keep your child out of the operating room, we think that is better for everyone.

 

This study provided a small number of children 1 tsp of lemon-flavored cod liver oil and ˝  tablet of a children’s chewable multivitamin formula each day. In this group of kids, they had a 12.3% fewer days of antibiotics as compared to the control group. While not earth shattering, this was statistically significant.
